Job OverviewWe are seeking dedicated and skilled Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our growing team in Galveston, Texas. The ideal candidate will provide high-quality patient care in skilled nursing facilities / nursing homes in collaboration with our team’s senior physicians. Part-time and full-time positions are available ranging from 1 day a week to 5 days a week.
Key Responsibilities and Requirements
Patient Care & Visits
- Conduct timely and thorough evaluations and treatments for new admissions and readmissions.
- Reliably perform follow-up visits based on practice guidelines and patient needs.
- Address acute changes in patient conditions, ensuring optimal care to prevent unnecessary hospitalizations.
Medical Competency & Treatment
- Accurately assess patient and develop management plans that are consistent with current standard of care and our group’s practice guidelines. (Practice guidelines and other reference resources will be provided by the practice upon hiring).
- Prevent hospitalizations by promptly addressing medical concerns such as infections, dehydration, volume overload, uncontrolled diabetes, acute kidney injury, and uncontrolled hypertension.
- Committed to lifelong learning.
Communication
- Reliably and promptly respond to calls, texts, and other requests from the skilled nursing facilities / nursing homes during required hours. There will be no overnight call duties.
- Dedicate an appropriate amount of time to patients and/or families during medical encounters to ensure comprehensive care.
- Clearly and effectively explain care plans to patients, families, and the directors of nurses at the skilled nursing facilities / nursing homes.
Professionalism & Teamwork
- Exhibit a courteous and professional attitude when communicating with collaborating physicians, nurses, other healthcare providers, patients, and families.
- Treat all parties with respect and demonstrate openness to constructive feedback to foster a positive working environment.
Documentation & Compliance
- Complete progress notes promptly in accordance with practice requirements.
Qualifications
- Active Nurse Practitioner license or Physician Assistant license in the state of Texas.
- Clinical experience as an NP or PA in one of the following settings: skilled nursing facilities / nursing homes, hospital (such as hospitalist team, medicine consult team, or medicine subspecialty consult team), primary care clinic, or medicine subspecialty clinic (such as cardiology or pulmonology).

Pay: $154,115.00 – $200,000.00 per year
