Nurse Practitioner Physicians Assistant
FAMILY NURSE PRACTITIONER OPPORTUNITY
Mansfield, TX

We are seeking an energetic and compassionate Nurse Practitioner or Physicians Assistant who enjoys variety in their day; someone passionate about health education and embodies the concept of wellness. Physicians and staff work very closely together to integrate all aspects of general family medicine, and provide the highest quality of medical services to their patients from children to adults. Focus is on preventive care to maximize a patient’s health and quality of life.
The Clinic is open 8am-5pm hours on Monday thru Friday.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
The Nurse Practitioner will be assigned to carry out functions of care as deemed appropriate by his/her supervising physician and in the strict accordance with the established protocols and procedures.
- Assesses, diagnoses and treats patients.
- Performs routine physicals (i.e. athletic, WIC, admission).
- Determines patient and family needs during initial assessment.
- Provides services in accordance with hospital and the transitional care clinic.
- Follows treatment protocols as established by medical director.
- Consults with medical director when needed regarding treatment plan.
- Recognizes and identifies life threatening situations, responding with immediate action according to protocol.
- Reviews all lab test and consults with medical director on abnormalities.
- Assures that adequate patient records are maintained and reviewed by medical director.
- When applicable, follows hospital protocol and physician orders regarding use of restraints.
- Utilizes referral services when needed.
- Identifies the need of the patient and discusses the plan of care with the patient and/or family.
- Consults with supervising physician and social worker to determine sources to refer patient.
- Arranges for, or refers patients to needed service that cannot be provided by the transitional care clinic.
- Communicates to the patient and/or family necessary actions needed to be taken to obtain desired results.
- Follows-up with patient or referring sources to ensure successful outcomes.
- Teaches and provides preventative care.
- Initiates teaching for patients and/or family members specific to their individual health care needs.
- Instructs patients during routine exams on importance of preventative health care with special emphasis on chronic disease self-management for diabetes and heart failure.
- Involves patient/family in development of realistic, specific and measurable short-term and long-term goals.
- Participates in health lectures for community groups as needed.
- Participates in health fair functions throughout the year as needed.
- Evaluates plan of treatment.
- Consistently modifies patient’s plan of treatment based on changes in patient’s condition as determined during follow up session.
- Actively participates in continuous quality improvement.
- Documents on records – patient’s needs, ability to manage continuing care needs, teaching, and referrals.
- Participates with the medical director in the periodic review of patients’ records.
EDUCATION / EXPERIENCE:
- At least 1 years working as a Nurse Practitioner or Physicians Assistant in an Urgent Care or Primary Care Setting
JOB RELATED EXPERIENCE:
- Minimum 1 years clinical experience in primary care or urgent care
