Full Job Description

CenterWell Senior Care Centers is seeking a Board Certified/Board Eligible Advanced Practice Provider for a position in Fort Worth, TX. This position requires the ability to conduct patient visits in Spanish and English.

A day in the life of a CenterWell provider:

  • See 12-15 patients per day. Opportunity to spend more time with patients
  • Thrive in a value-base care model and improve patient’s quality of life
  • See a variety in your caseload, from preventative care to chronic conditions
  • Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Your Accomplishments are Valued:

  • Active, unrestricted NP/PA license in the appropriate state
  • Current certification as an NP or PA
  • Current DEA license or ability to obtain a DEA license
  • This role is considered patient facing and is part of Humana/Senior Bridge’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • Bilingual English/Spanish

Preferred Qualifications:

  • Familiar with Value Based Care model
  • Outpatient Senior Care Experience
  • Familiar with HEDIS measures

Pay: $110,000.00 – $130,000.00 per year
