About the Role:We’re excited to offer a unique opportunity for a Physician Assistant to work across two innovative and patient-centered healthcare models: Elevate Brain Injury & Mental Health and MaxHealth.

At Elevate, we focus on diagnosing and treating patients with Mild Traumatic Brain Injuries (mTBI) and associated mental health conditions. At MaxHealth, we provide accessible and high-quality primary and urgent care, both in-person and via telemedicine.

This full-time role blends brain health and primary care—offering variety, autonomy, and professional growth in a collaborative, technology-forward environment.

Responsibilities Include:

• Evaluate and manage patients with mTBI, mood changes, cognitive concerns, and post-accident conditions
• Create and manage individualized treatment plans
• Coordinate care with internal team members and specialists
• Provide mental wellness and preventative care
• Conduct routine physicals and urgent care visits
• Manage chronic and acute conditions
• Order and interpret lab work and imaging
• Perform prescription management and preventative screenings
• Deliver care in person and via telehealth platforms
• Document assessments and progress notes in the EMR

What We’re Looking For:

• Licensed Physician Assistant in Texas
• Licensed Family Nurse Practitioner (FNP) in Texas are encouraged to apply
• Primary care or family medicine experience required
• Experience in urgent care, mental health, sports medicine, or brain injury is a plus
• Strong communication and interpersonal skills
• Compassionate, patient-centered approach
• Comfortable working in a hybrid model using telemedicine and digital tools
• Spanish bilingual candidates are strongly encouraged to apply
