OverviewWe are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of chronic conditions. This role requires a strong commitment to patient-centered care and the ability to work collaboratively within a multidisciplinary team.

Duties

  • Conduct thorough patient assessments, including physical examinations and medical histories.
  • Develop and implement individualized treatment plans based on patient needs.
  • Provide care in various settings, including home care, outpatient facilities, and urgent care environments.
  • Manage chronic conditions and perform discharge planning for patients transitioning from hospital to home.
  • Administer vaccinations, perform phlebotomy, and conduct diagnostic tests as necessary.
  • Educate patients and their families about health management, preventive care, and lifestyle modifications.
  • Collaborate with other healthcare professionals in teaching hospitals to ensure comprehensive patient care.
  • Address behavioral health issues and provide dementia care as part of holistic treatment approaches.
  • Utilize ICD-10 coding for accurate documentation and billing processes.
  • Participate in quality improvement initiatives to enhance patient outcomes.

ExperienceCandidates should possess the following qualifications:

  • A Master’s degree in Nursing with a specialization as a Family Nurse Practitioner.
  • Current state licensure as a Nurse Practitioner with national certification.
  • Experience in critical care or urgent care settings is preferred but not mandatory.
  • Familiarity with home care practices and discharge planning processes is advantageous.
  • Strong communication skills with the ability to teach patients effectively about their health concerns.
  • Proficiency in using electronic health records (EHR) systems for documentation purposes.
