We are seeking a compassionate and dedicated Primary Care Nurse Practitioner/ Physicians Assistant to join our healthcare team. The NP/PA will provide comprehensive, patient-centered care in a primary care setting, including preventive care, diagnosis, and management of acute and chronic conditions. The ideal candidate will work collaboratively with physicians and other healthcare providers to improve patient outcomes and ensure quality care.
Key Responsibilities:
- Conduct comprehensive health assessments, including patient history, physical exams, and diagnostic tests.
- Diagnose and manage acute and chronic illnesses.
- Develop and implement individualized treatment plans.
- Prescribe medications and therapies according to scope of practice and regulatory guidelines.
- Provide preventive care, including screenings, immunizations, and health education.
- Coordinate referrals to specialists and community resources when appropriate.
- Maintain accurate and up-to-date patient records using electronic health record (EHR) systems.
- Collaborate with interdisciplinary care teams to provide patient care.
- Participate in quality improvement initiatives and continuing education.
Qualifications:
- Education: Master’s or Doctorate degree in Nursing from an accredited Nurse Practitioner program.
- Licensure/Certification: Current and unrestricted Nurse Practitioner license (State-specific), board certification (AANP or ANCC), and DEA registration.
- Experience: Minimum of 1-2 years in primary care preferred; new grads considered based on clinical rotations and background.
- Skills: Strong clinical judgment, excellent communication, and ability to work independently and within a team.

Job Type: Full-time
Pay: $110,000.00 – $125,000.00 per year
