Job Summary:We are seeking an experienced Advanced Practice Providers to join our healthcare team. As an Advanced Practice Provider, you will provide comprehensive patient care and support in various healthcare settings. Your expertise and skills will contribute to the overall well-being of our patients.
Duties:- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic evaluations- Develop and implement individualized care plans based on patient needs and medical conditions- Administer medications, treatments, and therapies as prescribed- Monitor and evaluate patient progress, adjusting care plans as necessary- Collaborate with other healthcare professionals to ensure coordinated and holistic patient care- Educate patients and their families on disease management, treatment options, and preventive measures- Provide support and guidance to nursing staff, assisting with complex cases or procedures- Stay up-to-date with advancements in medical research, technology, and best practices
Experience:To be successful in this role, you should have the following qualifications:
- Must be a graduate and certified from an accredited advanced practice program
- Two or more years of outpatient clinical experience as an Advanced Practice Provider, with a demonstrated ability to work autonomously; capable of providing care without physician supervision
- Current, unrestricted license in practicing state or be eligible for licensure
- Active Drug Enforcement Agency (DEA) registration or be eligible for registration
- Must possess active Medicare/Medicaid Enrollment or be eligible to enroll
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
- Strong oral and written communication skills
- Comfortable with Electronic Medical Record (EMR) system; Athena preferred
- Familiarity with Microsoft Office
