Job Summary:
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) Or Physician Assistant (PA) to join our healthcare team. In this role, you will provide comprehensive, patient-centered care across the lifespan, emphasizing health promotion, disease prevention, and chronic disease management. The ideal candidate will have strong clinical skills, a passion for delivering quality care, and the ability to build lasting relationships with patients and their families.
Key Responsibilities:
- Patient Care: Conduct comprehensive health assessments, including patient histories, physical examinations, and diagnostic testing.
- Treatment and Management: Diagnose and manage acute and chronic illnesses, develop treatment plans, and prescribe medications as needed.
- Preventative Care: Provide education on disease prevention, lifestyle modifications, and overall wellness.
- Collaboration: Work closely with physicians, specialists, and other healthcare team members to ensure coordinated and effective care.
- Patient Education: Counsel patients and families on healthcare options, treatment plans, and follow-up care.
- Documentation: Maintain accurate and thorough medical records in compliance with regulatory and organizational guidelines.
- Quality Assurance: Participate in quality improvement initiatives to enhance patient outcomes and practice efficiency.
- Community Engagement: Contribute to community health initiatives and patient outreach programs when necessary.
Qualifications:
- Education: Master’s in Nursing or Physician Assistant Studies from an accredited program.
- Licensure: TN State License and Certification as an FNP or PA.
- Experience: Minimum of 1 year of experience as an FNP or PA (preferred but not required).
- Skills:
- Strong clinical and diagnostic abilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health record (EHR) systems.
- Dedication to patient-centered care and continuous learning.
- Certifications: Current BLS/ACLS certification required.
Work Environment:
- Outpatient clinic, hospital, or community health setting.
- Occasional evening or weekend hours may be required.
