Job Title: Nurse Practitioner (NP) or Physician Assistant (PA)– Nursing Home Care

Job Type: Full Time (4 or 5 days a week)

Location: NHC Post-Acute and Long-Term Care and Assisted Living Center in Franklin TN

Overview: We are seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality care for residents in our nursing home facility. The ideal candidate will collaborate with an interdisciplinary healthcare team to ensure comprehensive care for residents, improve health outcomes, and enhance the quality of life for individuals in a long-term care setting.

Responsibilities:

  • Conduct comprehensive health assessments, including physical exams, patient histories, and resident diagnostic evaluations.
  • Develop, implement, and evaluate individualized care plans in collaboration with residents, families, and the healthcare team.
  • Diagnose and manage acute and chronic illnesses, prescribing medications and treatments as appropriate.
  • Monitor and document residents’ progress and response to treatment plans, adjusting as necessary.
  • Provide preventative care and health education to residents and families, focusing on disease prevention and wellness promotion.
  • Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to coordinate and optimize resident care.
  • Respond promptly to medical emergencies and provide expert guidance in critical situations.
  • Ensure compliance with nursing home regulations, standards of care, and organizational policies and procedures.
  • Participate in staff education and training, offering guidance on evidence-based practices and care approaches.
  • Advocate for residents’ rights and dignity, ensuring a safe and supportive environment.

Qualifications

  • Current Nurse Practitioner or Physician Assistant license in Tennessee
  • Board certification in a relevant specialty (e.g., Adult-Gerontology, Family Practice).
  • A minimum of two years of experience in geriatrics, long-term care, or a similar setting is preferred.
  • Strong knowledge of geriatric care, chronic disease management, and palliative care.
  • Excellent interpersonal, communication, and organizational skills.
  • Proficiency in electronic medical records (EMR) and other relevant healthcare technologies.
