Full Job Description
Local SNFist group is hiring one NP or PA to provide care in the Post-Acute/Long Term Care (PA/LTC) setting in Chattanooga, TN .
This position is full-time, Monday-Friday. The provider will manage typical medical problems to assist patients in transitioning from the hospital to home.
Call is provided by a national telehealth group with the NP/PA available for back-up and oversight of any transfers to the hospital.
Experience in Post-Acute/Long Term Care (PA/LTC) is preferred, but other experience is welcome.
