Mazzoni Center is excited to recruit for a Physician Assistant / Certified Registered Nurse Practitioner to join our team!
Job Summary: Provides culturally competent primary care to patients of Mazzoni Center.
Responsibilities:
- Provide direct patient care to ambulatory patients in primary care setting – including assessment and treatment of acute and chronic medical problems, office-based procedures, patient education and counseling, and obtain medical histories.
- Notify/Consult with supervising physician regarding patient care as needed and refer to physician for more complicated cases or case that are not a routine part of the scope of CRNP/PA.
- Monitor therapies and ensure continuity of care.
- Triage patient calls and evaluate patient concerns. Participate in on call rotation and respond to emergencies, including use of CPR.
- Ensures complete and timely documentation of care in accordance with Mazzoni Center policies (being completed within 72 business hours after the provision of care).
- Coordinate Care with other providers/specialty of care (internal and external, including registered nurses, behavioral health consultants, care coordinators, case managers) to facilitate comprehensive care.
- Counsel patient/family on preventative care, initiate and supports efforts to promote wellness concepts and empowerment for health, especially retention in care for those with chronic illness.
- Review patient records to develop comprehensive medical status reports
- Responsible for all insurance, grant-base and internal quality measures for their panel of patients.
- Participate in grand rounds, M&M rounds, quality reviews and case conferences.
- Precept/Mentor leaners and engage in educational activities.
- Participate in department meetings and committees.
- Communicate with the Director of Practice Management in a timely manner regarding schedule changes, time off, approved expenses, licensure, and other matters relating the practice function.
- Perform other duties as assigned.
Education, Training, Licensure and Certification, Skills and Experience:
- Bachelors Degree and licensure of as a Physician Assistant or Certified Registered Nurse Practitioner in the Commonwealth of PA.
- Maintenance of certification/licensure as required by National Commission on Certification of PAs (if applicable)
- 5 + years of experience.
- Experience in Primary Care setting; comfortable with adult, pediatric, and primary care gynecology as well as LGBTQ focused care.
- Practice in accordance with Mazzoni Center’s mission and demonstrating commitment to the LGBTQ community.
- Engage in continuing medical education at least annually
- Literate with general principles of EMR and basic computer skills
- Ability to communicate effectively both orally and in writing.
