Key Responsibilities:
- Evaluate, diagnose, and manage acute and chronic illnesses, providing holistic patient care.
- Perform phlebotomy and other routine lab procedures, ensuring accurate collection and handling of specimens.
- Coordinate care with specialists, social services, and community resources to enhance patient outcomes.
- Deliver telehealth services as needed, expanding access to care for our patients.
- Participate in quality improvement initiatives and the development of innovative care models.
- Educate patients on managing their health conditions and empower them to achieve optimal wellness.
- Provide timely and accurate documentation in electronic health record (EHR) systems, ensuring confidentiality and compliance with regulations.
Skills & Competencies:
- Expertise in clinical decision-making, patient assessment, and phlebotomy techniques.
- Strong interpersonal skills with the ability to build trust and rapport with patients.
- Ability to adapt to new healthcare technologies and participate in continuous improvement efforts.
- Flexibility to accommodate varying patient volumes and clinic needs, especially for per diem shifts.

Requirements:
- Education: Master’s degree in Nursing with certification as a Nurse Practitioner or completion of an accredited Physician Assistant program.
- Licensure: Current Nurse Practitioner or Physician Assistant license in Pennsylvania.
- Experience: Minimum of 2 years in a clinical setting, with community health experience preferred. Phlebotomy experience is required.
- Proficiency: Comfortable using EHR systems and telehealth platforms.
- Commitment: Full-time practitioners should be available during regular clinic hours, while per diem staff must be open to covering a variety of shifts.
