Key Responsibilities
- Perform comprehensive wound assessments and create individualized treatment plans
- Provide advanced wound care in patient homes and care facilities
- Conduct telehealth visits as permitted by Oregon regulations
- Prescribe medications and treatments as clinically appropriate
- Document patient care accurately and in a timely manner
- Travel locally between patient locations (mileage reimbursed)
- Optional: Rounding in Skilled Nursing or rehabilitation facilities
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Oregon
- Master’s degree from an accredited NP or PA program
- Preferred: 1+ year of experience in primary care, urgent care, internal medicine, or wound care
- Bonus: ER, ICU, or other critical care experience
- Strong clinical judgment, communication, and organizational skills
- Compassionate, patient-centered approach
Compensation & Benefits
- Salary: Up to $165,000 annually (full-time, salaried)
