Integrated Primary Care + Mental Health | Collaborative Team Environment

Well Life Medicine, a thriving independently owned primary care and mental health clinic in Salem, Oregon, is seeking an experienced Physician Associate (PA) to join our growing multidisciplinary care team.

This is a unique opportunity to work in an independent, mission-driven practice where providers are valued, patients are known, and collaboration is part of the culture. We are looking for a provider with experience in family medicine, internal medicine, urgent care, or a similar primary care setting. Candidates should be comfortable managing a broad range of acute and chronic conditions and working closely with a team to deliver thoughtful, patient-centered care.

This role is ideal for an PA who wants to practice meaningful primary care without the bureaucracy of a large health system.

This position offers a consistent, set schedule:Monday–Thursday, 8:00 AM–6:00 PM (or can shorten hours if you prefer 32 hour work week)

No weekend or night shifts are required. Minimal on-call responsibilities are supported by an after-hours answering service (typically one week every 6-8 weeks, call-only, compensated).

Job Responsibilities

The PA will:

  • Provide direct primary care, including new patient visits, preventive exams, urgent care visits, and follow-up appointments
  • Diagnose and treat acute and chronic conditions across the lifespan, generally ages 5 and up (or can do 18+ only if background is internal medicine).
  • Provide preventive care, including annual exams, well-child visits, well-woman visits, and Pap smears
  • Create patient-centered, evidence-based treatment plans
  • Build meaningful, long-term relationships with patients
  • Collaborate closely with other providers, medical assistants, behavioral health clinicians, the Medical Director, and clinic leadership
  • Participate in regular case review and care coordination
  • Document patient encounters accurately and efficiently in the EHR
  • Provide occasional telehealth care when appropriate
  • Opportunity for “express clinic” or walk-in hours as a part of your daily schedule if you are interested.

This is an on-site position based in Salem, Oregon.

Patient Volume

Providers typically see 16–19 patients per day (in 10 hours), with 30-minute appointment blocks.

Qualifications

Required:

  • Licensed PA in Oregon, or eligible to obtain Oregon licensure within 90 days
  • At least 1 year of experience in family medicine, internal medicine, urgent care, or primary care
  • Ability to function autonomously (of course we have clinical assistance for complex patients)
  • Strong clinical decision-making skills across the primary care scope
  • Comfort treating patients ages 5 and up; adult-only primary care experience may be considered for the right candidate
  • Strong communication, documentation, time management, and organizational skills
  • Ability to work collaboratively in a team-based care environment
  • Commitment to culturally competent, patient-centered care
