We are looking for a full time Nurse Practitioner/Physician Assistant to join a team of professionals, who excel in providing unique health care for Seniors! The Program of All-Inclusive Care for the Elderly (PACE) Clinic team performs a vital service by providing primary care for Tulsa’s aging population by offering older adults and their caregivers a welcome alternative to nursing home care. Participants have access to comprehensive medical services and social support while they enjoy independent living in their own homes and communities.

The Nurse Practitioner/Physician Assistant is a hands-on clinic position that provides primary medical care to PACE program participants. Working in collaboration with the supervising physician, you will share a panel of participants by assessing health and medical conditions, diagnose and treat/prescribe within ARPN/PA scope of practice and assist in the development of a plan of care in collaboration with the interdisciplinary team (IDT), provision of direct participant medical care, and evaluation of the effectiveness of the plan of care. Serving as a member of the interdisciplinary team you will participate in morning meeting updates and provide updates to the team regarding changes in participant’s health or functional status. In this position you will be working exclusively with seniors, some with dementia, patience and extra compassion will sometimes be required.

In coordination with the supervising physician and within NP scope of practice, primary responsibilities include:

  • Acting as a physician extender in the event of episodic illness and managing chronic illnesses/conditions. Review consultation notes, place orders for follow-up visits as needed, evaluate and order medications as appropriate, and address nursing concerns/questions. Completion of charting and documentation in the electronic medical record in a timely fashion.
  • Assist participants in maintaining optimum quality of life, understanding that quantity of life will not likely be extended in this population; provide health education and counseling to participants and caregivers facing chronic conditions and end-of-life issues.
  • Be flexible in schedule of hours worked, although outside of on-call, hours are generally 8:30 am to 5:00 pm Monday through Friday. Participate in the clinical on-call rotation, shared with other PACE providers. Communicate with weekend and after-hours on-call staff, following up on issues as necessary. May require use of personal vehicle.
  • Adhere to and support the center’s policies, practices, and procedures. Provide follow-up for clinic complaints as specified in the LIFE PACE program grievance policies and procedures; document complaints per PACE program grievance policy. Participate in data collection, surveys and other duties as assigned. Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
  • Maintain confidentiality of participant, staff, contractor, and organizational information. Demonstrate a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the agency. Attend and successfully complete mandatory HIPAA/HITECH training at least annually.

Requirements:

EDUCATION, EXPERIENCE AND SKILLS:

  • Master’s degree from an accredited Physician Assistant or Nurse Practitioner program.
  • Must have a current and unrestricted NP and/or PA license to practice medicine in the state of Oklahoma with current DEA, OBNDD. Must provide copy of current state license, DEA registration and OBNDD.
  • Minimum of one (1) year of documented experience working with a frail or elderly population is required. Experience in an out-patient primary care clinic is preferred. Interest in frail older adults and knowledge of their physical, mental, and social needs. Effective skills in physical assessment and chronic disease management for frail older adults. Desire to work in a creative alternate health care setting.
  • Able to manage changing priorities according to participant needs. Ability to work effectively and in a collegial manner with all members of the interdisciplinary team. Strong organizational skills, dependable, flexible, and resourceful. Effective oral communication skills.
  • Spanish bilingual skills strongly preferred.
  • Reliable personal vehicle and valid driver’s license.
