Job Description: Wound Care Nurse Practitioner or Physician Assistant
Essential Functions:
1. Develop and manage an individualized wound care treatment plan for each patient to offer
optimal healing outcomes or chronic maintenance.
2. Collaborates closely with facility wound care nurse, home health/hospice nursing staff, and facility
Wound Care Staff regarding wound management practices.
3. Respond timely to calls or questions from any Wound Care staff. Must have open lines of
communication.
4. Adhere to any and all facility Wound Care policies and procedures.
5. Knowledge of up-to-date topical wound care products and therapies, including ordering, applying, and management of the products on wounds, also including negative pressure wound therapy.
6. Provides hands-on wound care to patients including debridement of wounds by enzymatic, sharp,
or autolytic methods when appropriate.
7. Order, review, interpret, and integrates diagnostic studies and laboratory findings for initiating or
modifying the treatment plan.
8. Have the ability to operate all assigned electronic devices and equipment. Utilize all features
within the EMR (fax, print, upload documents)
9. Closely follow the clinical pathways, order laboratory testing, vascular studies, or durable medical
equipment (wedges, mattresses, or other equipment) at initial visit unless already noted in chart.
10. Collaborate with the patient’s primary care provider when comorbidities are contributing to or
affecting the healing of a wound to determine a comprehensive plan.
11. Prescribes medications in a timely fashion and discuss compliance and reasoning for medication
to patient or family.
12. Provides the required billing information for services provided by the Wound Care NP and maintain
the documentation necessary for those services billed within 48 hours of appointment.
13. Review patients prior medical records and maintains their current medical record related to the
wound diagnosis and contributing comorbidities.
14. Request additional information as needed from previous medical events or encounters.
15. Orders are to written, signed, and faxed or given to facility DON, home health or hospice at
bedside. Followed by calling or speaking to the DON or charge nurse to ensure receipt of orders and discuss the plan of care.
16. Patients chart encounter will be closed according to the standard of care. Within 48 hour all charts
are to be signed, locked, and ready for billing.
17. Update the DON, QA, and scheduling of any missed visits. Any changes to the daily schedule will
be reported on SharePoint and communicating with the DON.
18. Assist with the identification of wound care quality measures, and provide data and
documentation for tracking of quality measures.
19. Instruct patient and family regarding wound treatments and management plan for optimal wound
outcomes. Discuss the plan of care, orders, medications, wound healing rate, and predicted
outcomes at each visit.
20. Triages telephone calls and provide information as requested by the patient, patients family,
facility DON, home health, or hospice. These calls will be addressed on the day they are received. 21. Participates in professional development activities and maintains professional affiliations. Assist
with marketing events or in-services to help build relationships with facilities or referral sources. 22. Completes required number of continuing medical education (CME) hours for license and
certification. Including all assigned educational modules in Wound Reference within time allotted. 23. Attends required meetings or committees as requested. (Case conference, educational lectures,
and mandatory meetings, ect)
24. Maintains patient confidentiality at all times.
25. Performs other duties as determined by leadership team.
26. Maintains a copy of all professional records including current licensure, certification, continuing education, and other appropriate records to practice within the state. A copy of these will be obtained for the providers digital portfolio.
27. Responsible for uploading any and all documents for required licensure or credentialing.
28. Report immediately any changes in status of any licensure or credentialing status.
29. Renewal of all licensures, certifications, and other documents required by all states, federal, local
and Total Wound Care guidelines prior expiration or renewal date.
30. Maintain all assigned equipment, devices, and vehicle in good working order.
Physical/Mental Demands and Work Environment:
1. Exposure to communicable diseases, medical prescriptions, toxic substances, and other
conditions. Including homes with pets, animals, and/or insects/bugs.
2. Requires full range of body motion to get in and out of vehicle, enter patients homes or facility. 3. Frequent standing, walking, bending, squatting, and sitting is required for extended periods. 4. Ability to maintain proper body mechanics while performing job duties. Report any injuries or
incidents to Human Resources as discussed in the policy handbook.
5. Requires visual and hearing acuity.
6. Able to lift a minimum of 35 pounds repetitively during a scheduled day.

Qualifications
- Valid Nurse Practitioner license
- Proficient in patient care and assessment
- Experience with EHR systems
- Knowledge of physiology and critical care
- Familiarity with medical office operations
Job Type: Full-time
Pay: $130,000.00 – $140,000.00 per year
