Job Summary:
The Advanced Practice Registered Nurse / Physician Assistant (Provider) delivers medical care to Valir PACE participants. The Provider works jointly with physicians, therapy, activities, dietary, social services and transportation as part of the Interdisciplinary Team to determine and monitor the care plan for each PACE participant in compliance with all national and state regulations/laws.
Duties/Responsibilities:
- Performs comprehensive exams on all participants upon enrollment then every six months and upon change of status in accordance with CMS regulations.
- Diagnose and treat acute illness in participants.
- Provide follow up care to manage chronic disease in participants.
- Engage the participants support system in medical care.
- Rounds/attends to institutionalized participants.
- Coordinates care of specialty providers, supporting good communication and providing follow-up indicated by specialty consults.
- Assist the IDT with the development and revision of care plans
- Serves on committees as required by the Medical Director, Program Director and/or Board of Directors.
- Utilizes good mannerisms and professional demeanor with participants, families, community care providers and PACE team.
- Ensures positive health outcomes for participants through vigilant examinations, specialty care when needed and medication management.
- Carries out accurate and timely documentation.
- Other duties as assigned.
Education, Licenses, Certifications and Experience
- Current APRN or PA License
- Certified to practice in the State of Oklahoma without limitation or restriction
- Licensed by both the Oklahoma Bureau of Narcotics and Dangerous Drugs Control (OBNDDC) and the Drug Enforcement Agency (DEA)
- Either one year working with the frail and elderly population or approved training.
- Current CPR certification
