Position Summary:Optimum Care Hospital is seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in providing exceptional at-home healthcare services. The NP/PA will be responsible for delivering in-home medical care, conducting assessments, managing treatment plans, and offering ongoing support to patients with various health conditions. This role combines in-person visits and telehealth interactions to offer a holistic, patient-centered approach to care.
Key Responsibilities:
- In-Home Patient Assessments: Perform physical exams, review medical histories, and assess patient needs during in-person visits to ensure comprehensive care.
- Diagnosis and Treatment: Diagnose and manage acute and chronic medical conditions, order diagnostic tests, and create individualized treatment plans for each patient.
- Patient Education: Educate patients and their families on health management, medication adherence, lifestyle modifications, and prevention strategies.
- Care Coordination: Collaborate with physicians, nurses, and other healthcare professionals to provide integrated, multidisciplinary care.
- Prescription Management: Prescribe medications, monitor patient progress, and make necessary adjustments to medication regimens.
- Telehealth Consultations: Conduct follow-up appointments via telemedicine platforms for ongoing care, monitoring patient progress, and adjusting treatment plans as needed.
- Documentation: Maintain detailed and accurate electronic health records (EHR) for all patient encounters, including assessments, treatment plans, and progress notes.
- Emergency Response: Be available to address patient emergencies and escalate care to appropriate settings when necessary.
- Quality Improvement: Participate in quality assurance programs, audits, and patient care evaluations to ensure continuous improvement of services.
Qualifications:
- Education: Master’s degree in Nursing or Physician Assistant Studies.
- Licensure & Certification:
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license.
- Board certification in your specialty (Family, Adult-Geriatric, or other relevant certifications).
- Experience: Minimum of 2 years of clinical experience in a healthcare setting, with preference for those experienced in home healthcare, geriatrics, or chronic care management.
- Skills:
- Excellent clinical assessment and diagnostic skills.
- Strong communication and interpersonal skills to interact effectively with patients and healthcare teams.
- Ability to work independently and manage patient care autonomously.
- Proficiency with telehealth platforms and Electronic Health Records (EHR).
- Physical Requirements: Ability to travel to patients’ homes as needed and perform in-home assessments. Must be able to lift and carry necessary medical equipment.
Additional Requirements:
- Ability to work flexible hours, including occasional evenings or weekends as patient needs dictate.
- Valid driver’s license and reliable transportation for in-home visits.
