Job Summary:
Optimum Care Partners is seeking a compassionate and skilled Nurse Practitioner (NP)/ Physicians Assistant (PA) to provide high-quality healthcare to patients in the comfort of their own homes. The ideal candidate will be responsible for assessing, diagnosing, and managing a variety of health conditions, particularly for patients with chronic illnesses or those requiring post-hospitalization care. This role offers an opportunity to make a significant impact by delivering personalized, patient-centered care in a home-based setting.

Key Responsibilities:

  • Patient Care & Clinical Assessment:
  • Conduct comprehensive health assessments for patients in their homes, including physical exams, health histories, and evaluations of ongoing medical concerns.
  • Diagnose and treat a range of acute and chronic conditions, managing multiple medical issues concurrently, and providing personalized treatment plans.
  • Order and interpret diagnostic tests (e.g., lab work, imaging) and coordinate follow-up care as necessary.
  • Prescribe medications, treatments, and therapies based on clinical evaluations and current medical guidelines.
  • Provide health education to patients and their families about managing medical conditions, medications, lifestyle changes, and preventative care.
  • Chronic Disease Management:
  • Manage chronic conditions such as diabetes, hypertension, heart disease, COPD, and other long-term health issues.
  • Monitor patient progress and modify care plans as needed to ensure optimal patient outcomes.
  • Provide ongoing support and education to patients and families, focusing on disease prevention and health promotion.
  • Care Coordination:
  • Collaborate with interdisciplinary healthcare teams, including physicians, case managers, and social workers, to ensure coordinated care for patients.
  • Refer patients to specialists or other healthcare providers when necessary and ensure proper follow-up care.
  • Communicate with families and caregivers to ensure they understand the patient’s care plan and how to best support the patient at home.
  • Documentation and Compliance:
  • Maintain accurate, up-to-date medical records in compliance with HIPAA regulations and organizational policies.
  • Document patient interactions, assessments, treatments, diagnoses, and follow-up plans in the Electronic Health Record (EHR).
  • Ensure proper billing and coding for services provided and collaborate with billing departments to ensure accurate patient invoicing.
  • Acute Care and Emergency Management:
  • Respond to acute health needs or emergencies during home visits, providing immediate care, triaging, and arranging for transport if necessary.
  • Educate patients and families about when and how to seek emergency care and make available resources for urgent needs.
  • Patient and Family Education:
  • Educate patients and families on effective management of chronic diseases, medication adherence, and preventive health strategies.
  • Provide guidance on post-hospital care, rehabilitation, and lifestyle modifications aimed at improving long-term health outcomes.

Qualifications:

  • Education:
  • Master’s from an accredited Nurse Practitioner/ Physician’s Assistant program.
  • Licensing/Certifications:
  • Current and valid Nurse Practitioner (NP)/ Physicians Assistant license in the state of practice.
  • Board Certification as a NP/ PA
  • Basic Life Support (BLS) certification is required.
  • Advanced Cardiovascular Life Support (ACLS) certification is preferred but not required.
  • Experience:
  • At least 2 years of experience as a Provider in a primary care, urgent care, home health, or hospital setting- Preferred.
  • Experience managing chronic diseases and delivering home-based care is preferred.
  • Skills:
  • Strong diagnostic and clinical assessment skills with the ability to develop and implement effective care plans.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare teams.
  • Ability to work independently while collaborating with an interdisciplinary team.
  • Proficiency in Electronic Health Records (EHR) systems and telemedicine tools.
  • Strong organizational skills and the ability to manage multiple patient cases simultaneously.

Physical Demands:

  • Ability to travel to patient homes, requiring reliable transportation (mileage reimbursement provided).
  • Ability to perform physical assessments and assist patients with mobility or other tasks as necessary.
  • Occasional lifting or moving of patients or equipment may be required.

Pay: $100,000.00 – $120,000.00 per year
