Job Description:

We are seeking a highly motivated and compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to join our dynamic Physical Medicine & Rehabilitation (PM&R) team. In this role, you will work collaboratively with our experienced PM&R physician to provide high-quality inpatient rehabilitation care to patients recovering from a variety of conditions, including stroke, spinal cord injury, brain injury, and other complex medical and neurological conditions.

Key Responsibilities:

· Conduct comprehensive patient assessments and develop individualized care plans in collaboration with the PM&R physician.

· Perform PMR evaluations, consultations and follow up visits.

· Order and interpret laboratory and imaging studies when clinically appropriate.

· Provide patient and family education on treatment plans, rehabilitation goals, and discharge planning.

· Monitor and document patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.

· Coordinate care with multidisciplinary teams, including physicians, physical therapists, occupational therapists, speech therapists, social workers and other team members as needed.

· May participate in team rounds, case conferences, and quality improvement initiatives.

· Timely documentation of all patient interactions with the use of EMR (documentation may be done on or off site).

Qualifications:

· Certified Physician Assistant (PA) or Nurse Practitioner (NP) with a valid state license.

· Minimum of one year of clinical experience preferred.

· Effective and efficient problem-solving skills.

· Excellent communication and interpersonal skills, with a commitment to patient-centered care.

· Ability to work collaboratively within a multidisciplinary team environment.

· PA-C or FNP certified by their respective board.

· Proficiency in electronic medical record (EMR) systems.
