CVOR Thoracic PA

Job Overview: This position is a clinician with advanced education and clinical practice. Under direct physician supervision, as required byt the State Medical Board, this position provides primary health care and case management, focusing on disease prevention, wellness, and management of minor, chronic, and acute illness in collaboration with the PCPs. This position also works in close collaboration to establish realistic goals and a thorough participant plan of care. Physician Practices/Senior Services: May manage patients by providing primary health care and case management, focusing on disease prevention, wellness, and management of minor, chronic, and acute illness. Provides holistic care, taking into account the needs and strengths of the whole person. Collaborates with clients and other interdisciplinary health care members to establish realistic goals and activities to guide both the client and the provider in implementing care.
