REBIRTH ADVANCED HEALING

Rebirth Advanced Healing is a Physician-owned mobile Advanced Wound Care medical practice dedicated to providing compassionate, high-quality care to patients with complex wounds. When traditional wound care approaches aren’t enough to heal, our advanced approach combines mobile delivery with biological therapies, including amniotic grafts, to promote faster healing, reduce risks, and make recovery more comfortable.

Position Overview

We are seeking a highly motivated and skilled Nurse Practitioner or Physician Assistant to join our 1099/IC Provider team near Cleveland, OH. The ideal candidate will have a passion for wound care, and a commitment to providing exceptional patient care. Wound care experience is preferred, but not required, as training is available for the right candidate.

Responsibilities

• Provide comprehensive advanced wound care to patients in their homes, skilled nursing facilities, or assisted living facilities
• Conduct thorough patient assessments and develop individualized treatment plans
• Perform wound debridement, graft-laying, dressing changes, and other related procedures as needed
• Provide education to patients and families about wound care, healing, and prevention
• Collaborate with other healthcare professionals to ensure continuity of care
• Maintain accurate patient records and thorough documentation within required timelines
• Adhere to all applicable laws and regulations

Requirements

• Certified Nurse Practitioner or Physician Assistant with an active and unrestricted license in Ohio
• Excellent documentation, clinical skills and judgment
• Strong communication and interpersonal skills
• Wound care experience preferred; in-home experience a bonus
• Reliable transportation
• Ability to travel up to 1 hour (+/-) from your home to treat patients
• Ability to work independently, and as part of a team
• Ablity to maintain consistent weekly schedule
