Overview
We are seeking a dedicated and skilled Family Medicine Nurse Practitioner to join our healthcare team. The NP will provide primary care services, including assessment, diagnosis, treatment, and health education, to patients of all ages. This role emphasizes preventive care and chronic disease management in a supportive, team-based environment.

Duties

  • Conduct comprehensive patient assessments, including medical histories and physical exams
  • Diagnose and manage acute and chronic illnesses
  • Order, perform, and interpret diagnostic tests
  • Develop and implement patient-centered treatment plans, incorporating evidence-based practices
  • Prescribe medications and therapies in accordance with state regulations
  • Provide preventive care, including screenings, immunizations, and health counseling
  • Collaborate with physicians and other healthcare professionals
  • Maintain accurate and timely medical records
  • Utilize electronic health record (EHR) systems like Medent, Epic, and HealthElink for accurate medical documentation and coding (ICD-10/ICD-9/CPT).
  • Engage in telehealth consultations to expand access to care while maintaining HIPAA compliance and excellent patient communication.
  • Provide health coaching and education on topics like family planning, nutrition, memory care, and disease prevention to empower patients in their wellness journeys.

Qualifications

  • Master’s or Doctoral degree in Nursing (NP program)
  • Current, active Nurse Practitioner license in NY state
  • Board certification in Family Practice (FNP-BC or FNP-C)
  • DEA license (or eligibility)
  • Demonstrated experience working in diverse clinical environments such as hospital medicine, urgent care, outpatient clinics, or assisted living facilities is preferred.
  • Proficiency with EMR/EHR systems including Medent, Epic, and HealthElink.
  • Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and diagnostic evaluation techniques.
  • Ability to work with diverse populations .
  • Excellent communication skills for triage assessments, patient education and case management; experience in telehealth also.

Pay: $100,000.00 – $140,000.00 per year
