Location: Brooklyn, NY (Canarsie)Schedule: Saturday & SundayCompensation: $500 – $600 per day (1099)
Genesis Medical Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide weekend primary care coverage for geriatric patients in a skilled nursing facility (SNF) located in the Canarsie section of Brooklyn, NY.
This position is ideal for a provider who prefers weekend work and is comfortable managing geriatric patients in a nursing home / long-term care environment. The provider will manage patient visits, follow-ups, and coordinate care with facility staff to ensure continuity of care throughout the weekend.
Schedule
The provider will work weekends (Saturday and Sunday) and must remain available between 8:00 AM and 6:00 PM, with flexibility in structuring the day based on patient volume and facility needs.
Compensation
- $500 – $600 per day
- 1099 Independent Contractor
- Additional earning potential depending on patient volume
Clinical Responsibilities
- Provide primary care services to geriatric nursing home residents
- Conduct routine and urgent patient visits
- Manage chronic conditions and acute medical issues
- Coordinate care with nursing staff and facility leadership
- Participate in care management programs including CCM and RPM
- Document patient encounters in facility EMR systems such as SigmaCare or PCC
Requirements
- Active New York NP or PA license
- Active Medicare and Medicaid provider ID preferred
- Experience working with geriatric patients preferred
- Skilled Nursing Facility (SNF) experience preferred
- Strong clinical judgment and ability to work independently
- Nursing home or long-term care experience is highly valued
