Job Overview:We are seeking a dedicated Nurse Practitioner to provide comprehensive healthcare services within a nursing home setting. This role offers an excellent opportunity for new graduates eager to develop their clinical skills in a supportive environment. The Nurse Practitioner will collaborate with interdisciplinary teams to deliver high-quality, patient-centered care, ensuring the well-being of residents across diverse age groups and medical conditions. The ideal candidate will possess strong clinical assessment abilities, excellent communication skills, and a commitment to advancing senior and long-term care.
Duties:
- Conduct thorough physical examinations and health assessments of nursing home residents, including vital signs, medical history, and functional status evaluations.
- Develop and implement individualized care plans in accordance with best practices and regulatory standards.
- Manage acute and chronic conditions, including medication management, IV infusions, wound care, and pain control strategies.
- Perform procedures such as suturing, catheterizations, spinal taps, and dermal filler injections when necessary.
- Collaborate with physicians, nurses, therapists, and ancillary staff to coordinate patient care and discharge planning.
- Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks for documentation, order entry, and medical records management.
- Participate in quality improvement initiatives, infection control protocols, and compliance with HIPAA regulations.
- Engage in telehealth consultations when appropriate to extend care access.
- Assist with specialized treatments including botulinum toxin therapy for spasticity or esthetic laser procedures as part of holistic resident care.
- Provide education on health coaching topics such as nutrition, medication adherence, and disease management tailored to elderly populations.
Experience:
- Master’s Degree in a Nurse Practitioner Program: Family or Adult Geriatrics
- Current New York State License as a Nurse Practitioner.
- National Board Certification require
- American Association of Nurse Practitioners (AANP)
- American Nurses Credentialing Center (ANCC)
- Oncology Nursing Certification Corporation (ONCC)
- Bilingual (Spanish Speaking) is a plus
- EMR/EHR experience a plus
