Responsibilities
- Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans.
- Manage acute and chronic illnesses across various populations, including pediatrics, geriatrics, and adult patients.
- Administer treatments such as Botulinum toxin injections for aesthetic or therapeutic purposes, dermal fillers, and other esthetic laser treatments.
- Perform procedures including suturing, IV insertions, catheterizations, spinal taps, phlebotomy, and sterile processing techniques.
- Utilize EMR (Electronic Medical Records) systems such as Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently.
- Coordinate care through case management, discharge planning, and utilization review to ensure optimal patient outcomes.
- Engage in telehealth consultations to expand access to care while maintaining compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams in settings such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, outpatient clinics, and inpatient units.
- Provide health coaching on nutrition, medication administration, disease prevention, and lifestyle modifications.
- Support infection control protocols and participate in clinical research or quality improvement initiatives when applicable.
Qualifications
- Valid Family Nurse Practitioner license with certification from an accredited program; current state licensure required.
- Proven experience in primary care or specialized fields such as gastroenterology, critical care, emergency medicine and infectious disease is highly desirable.
- Demonstrated proficiency with EMR/EHR systems like Epic or Athenahealth; experience with CPT coding (Current Procedural Terminology) and ICD coding (International Classification of Diseases) is preferred.
- Strong clinical skills including suturing, IV infusion/insertion, catheter care, airway management, and basic life support certification.
- Knowledge of pharmacology related to psychopharmacology, pain management (including acute pain management), hospice & palliative medicine practices.
- Experience working with diverse populations including children (pediatrics), seniors (geriatrics), individuals with disabilities or developmental disabilities; familiarity with assisted living or memory care settings is a plus.
- Ability to perform diagnostic evaluations using laboratory testing (including specimen collection/processing), medical imaging interpretation, sonography skills, and physical examinations.
- Excellent communication skills for patient education and collaboration within multidisciplinary teams; commitment to maintaining HIPAA compliance and medical documentation standards. Pay: $130,000.00 – $150,000.00 per year
