Your Role and Key Responsibilities

As an Advanced Practice Provider (APP) at CareBridge Partners, you will play a vital role in delivering transformative care to patients who need you the most. In this hybrid role, you’ll combine in-home and virtual visits to provide comprehensive, patient-centered care while collaborating with an interdisciplinary team to reduce suffering and achieve the best outcomes.

Patient-Centered Care:

  • Conduct comprehensive assessments during initial visits and develop personalized care plans in collaboration with your team for patients with complex and chronic conditions.
  • Provide ongoing follow-up care, primarily through virtual visits, with occasional in-home visits as needed.
  • Clinical autonomy to directly diagnose and manage acute and chronic conditions, prescribing medications and therapies tailored to individual needs.
  • Educate patients and caregivers on disease management, medication use, and advanced care planning.

Collaboration and Teamwork:

  • Partner with a dedicated care team, including a Clinical Pharmacist, Certified Medical Assistant (Health Concierge), and LCSW, to address medical, behavioral, and social needs.
  • Serve as the clinical leader in team discussions, care plan reviews, and interdisciplinary meetings to align strategies and enhance patient outcomes.

Quality Improvement and Innovation:

  • Contribute to the development of best practices and clinical protocols to improve care delivery.
  • Engage in quality improvement initiatives aimed at reducing hospitalizations and enhancing patient care.
  • Provide feedback and insights to refine CareBridge’s clinical model and support value-based care initiatives.

Operational Excellence:

  • Maintain accurate documentation of patient care and progress using healthcare technology systems.
  • Ensure compliance with healthcare regulations, standards, and CareBridge Partners’ policies.
  • Communicate effectively with patients and care team members to support seamless, continuous care.

About You

We’re looking for a mission-driven Nurse Practitioner or Physician Assistant who desires to have clinical autonomy within in a team-based environment and is passionate about delivering transformative care to decrease the suffering of patients with complex medical needs.

Qualifications

Education:

  • Master’s or Doctorate in Nursing from an accredited program for Nurse Practitioners.
  • Master’s degree from an accredited Physician Assistant program for PAs.

Licensure: Active and unrestricted Nurse Practitioner or Physician Assistant license in North Carolina.

Experience: Minimum of 2 years in acute care, home health, primary care, geriatrics, or similar settings.

Certifications: National board certification required.

Skills

  • Strong clinical assessment, diagnostic, and management abilities based on evidence-based care.
  • Excellent communication and interpersonal skills for a hybrid care environment.
  • Proficiency in healthcare technology, including electronic medical records and telehealth platforms.
  • Ability to work independently while collaborating effectively within an interdisciplinary team.
  • A commitment to continuous learning and contributing to quality improvement initiatives

Pay: $125,000.00 – $175,000.00 per year
