Job Summary

The Duke CAREHub, comprising the DUHS Transfer Center and Patient Placement. The Transfer Center is crucial for patient access both locally and globally. The department uses real-time capacity data to facilitate access to specialty services and manage emergent needs. The Transfer Center APP (Nurse Practitioner or Physician Assistant) provides leadership within the care hub and works directly with the Transfer Center Medical Director and Transfer Center Nurses. Responsibilities include but are not limited to:

  • Performs Administrative Reviews of transfer center requests in real-time to decrease intake to response time.
  • Reviews same-day direct admissions in times of high hospital capacity as decided by CARE Hub leadership team.
  • Engages clinical teams when transfer requests require multiple services.
  • Responsible for conferencing appropriate transfer back patients.
  • Serves as an escalation provider for Clinical Expeditor.
  • Functions for Duke University Hospital (DUH).

Job Eligibility Requirements

Education

PA: Graduate of ARC accredited Physician Assistant program

NP: Graduate of accredited MSN or DNP Nurse Practitioner program for adult care.

Certification

PA: PA-C

NP: AGNP or AGACNP or ACNP or FNP

Licensure

PA: Current unrestricted license issued by the NCMB with approval to practice as PA

NP: Current unrestricted RN license issued by or recognized by the NCBON with approval to practice as an NP.

Experience

PA: Minimum 1+ years experience as a Physician Assistant is strongly suggested

NP: Minimum 1 + year experience as an NP or an equivalent combination of relevant education and/or experience is strongly suggested.

Physical and mental requirements

  • Able to clearly articulate scope of practice and practices within those guidelines as an NP/PA in the state of North Carolina and within the Duke University Health System (DUHS)
  • Able to independently seek out resources and work collaboratively to solve complex problems
  • Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others
  • Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up
  • Able to use fine motor skills
  • Able to record activities, document assessments, and interventions; prepare reports and presentations.
  • Able to use computer and learn new software programs
  • Able to provide leadership in clinical area of expertise and in meeting organizational goals
  • Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations
  • Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds
  • Able to remain focused and organized
  • Able to work collaboratively with all levels of personnel

Job Hours

Primary hours will be Monday-Friday, 10 hours a day with 1-2 weekends per month.
