Role Overview
We are seeking a Wound Care Nurse Practitioner (NP) to join our growing team. This role is ideal for a clinician who thrives in autonomous settings, values patient-centered care, and is passionate about improving outcomes for individuals with acute and chronic wounds.
Responsibilities
● Perform comprehensive wound assessments and develop individualized treatment plans
● Provide advanced wound care including debridement, dressing changes, and infection
management
● Collaborate with physicians, caregivers, and facility staff to coordinate care
● Document visits and treatment plans accurately in EMR systems
● Educate patients and caregivers on wound prevention and care
● Travel to patient locations within Livingston and nearby communities
Qualifications
● Active Nurse Practitioner license in New Jersey
● Certification in Wound Care (WOCN, WCC, or equivalent) preferred
● Minimum 1 year of wound care experience (home health or facility-based a plus)
● Strong clinical judgment and ability to work independently
● Excellent communication and documentation skills
● Valid driver’s license and reliable transportation

Pay: $80.00 – $85.00 per hour
