Care2U is a fast-growing healthcare startup providing on demand, in-home, urgent and acute care for older adults. Our goal is to keep patients in their homes and out of the hospital.

This job posting for a Physician Assistant (PA) or Nurse Practitioner (NP) at Care2U focuses on providing urgent and acute in-home care on an On-Call basis, ideal for healthcare professionals who value patient-centered, flexible work in Long Island. Here are the main aspects:

Job Responsibilities:

  • Home Visits: Conduct in-person home visits to manage acute illnesses and injuries.
  • Treatment & Collaboration: Provide treatment in line with evidence-based guidelines and collaborate with a multi-disciplinary team, including telehealth Emergency Physicians and care coordinators, to handle complex cases.
  • Risk Assessment: Identify risk factors, such as medication compliance, limited mobility, impaired nutrition, fall safety, and social isolation.
  • Documentation & Compliance: Maintain patient confidentiality, complete timely electronic records, and adhere to acute care standards.
  • Vehicle Requirements: Respond on an on-call basis using a personal vehicle, with the option to use a Care2U vehicle when available, and pick up necessary equipment from designated locations.

Requirements:

  • Education: Master’s degree in PA or NP studies from an accredited institution.
  • Licensure: Valid PA or NP certification and New York license.
  • Experience: Prior experience in emergency or hospitalist care; experience in geriatrics, hospital-at-home, or post-acute care is a plus.
  • Certification: BLS certification required.

Physical Demands:

  • Daily use of PPE and electronic documentation.
  • Frequent, prolonged standing, walking, and patient/equipment transfers.

Compensation and Job Type:

  • On-Call Basis: $75.00 – $80.00 per hour, with three hours of pay for the first patient and two hours per additional patient (most responses average 75 minutes). Responses as needed from 8am to 10pm.

  • Inquire about Traditional Per Diem Shifts: 8, 10, 12 hr shifts

Medical Specialties:

Schedule:

  • On-call coverage with responses required from your home in Long Island.
  • Once monthly scheduled shift to maintain proficiency.
  • Attendance at staff meetings
