Nurse Practitioner (NP) / Physician Assistant (PA) – Full-Time

We are actively hiring experienced Nurse Practitioners (NPs) / Physician Assistants (PAs) to provide comprehensive care across private Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALF) throughout the Long Island area.

Responsibilities
- Conduct thorough patient assessments and develop individualized care plans
- Diagnose, manage, and treat both acute and chronic conditions
- Monitor patient progress and adjust treatment plans as needed
- Document all visits using Electronic Health Record (EHR) systems
- Collaborate with physicians, nursing staff, and interdisciplinary teams
- Educate patients and families on health management and wellness strategies
- Maintain compliance with all healthcare regulations and HIPAA standards
Skills & Competencies
- Strong diagnostic and clinical judgment skills
- Proficient in medication management and post-acute care protocols
- Familiarity with infection prevention and chronic disease management
- Excellent communication, teamwork, and patient education skills
- Highly organized and efficient in managing multiple patients daily
- Comfortable working independently within facility settings
Qualifications
- Licensed and Board-Certified NP or PA in New York State
- Authorized to work in the United States
- Previous experience in Skilled Nursing, Rehabilitation, or Long-Term Care (preferred)
- Strong work ethic and commitment to high-quality care
- Proficiency with EHR systems (e.g., Practice Fusion, PointClickCare, or similar)
- Fluent in English (additional languages a plus)
