Salary Range: $114,000-$125,000 (Pay information will vary depending upon relevant experience for the position) Full Time 40 hours Monday-Friday Days

GENERAL SCOPE: Assists in the provision of comprehensive medical care for residents of the Jewish Home of Rochester. The Advanced Practice Provider provides comprehensive medical care for assigned long-term care (LTC) and short-term rehabilitation program (SRP) residents in collaboration with the Attending Physician and interdisciplinary care team. The Advanced Practice Provider is a valued member of the interdisciplinary care team reporting to the Vice President of Medical Services. The Advanced Practice Provider works collaboratively with residents, families, staff, and external providers and services to ensure excellent person-centered medical care.

EDUCATION / QUALIFICATION: Physician Assistant: Master’s degree or above; NYS Licensure. Nurse Practitioner: Master of Science in Nursing (MSN) with Gerontology/Family/Adult primary care specialization.

ESSENTIAL FUNCTIONS:

  • Provide high quality medical care for Jewish Home residents in collaboration with physicians and interdisciplinary care team
  • Provide communication, support and education to residents and their families in conjunction with the interdisciplinary care team
  • Knowledgeable regarding principles of palliative and end of life care
  • Aware of state and federal guidelines concerning nursing home medicine
  • Committed to the continuous quality improvement
  • Complete documentation and billing for visits in electronic record in a timely fashion
  • Participate fairly in evening, weekend, and holiday on-call coverage
  • Serve on committees and work groups as appropriate
  • Support and ensure residents’ rights
  • All other job duties as assigned.

EXPERIENCE: Two years or greater preferred, with experience in geriatrics, post-acute care, and long- term care preferred.
