Mobile Nurse Practitioner or Physician Assistant (NP or PA)
Location: Queens, NY (Field-Based – Travel Required to Multiple Practices)
Salary Range: $150,000–$175,000 annually (based on experience)
About the Role
StreamXCare is seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to join our growing interventional vascular and women’s health network across Queens. This hybrid mobile role is ideal for a provider who is confident working independently across multiple office locations and partner practices.
You will travel between collaborating medical offices and surgical centers to deliver on-site patient evaluations, support interventional procedures, and coordinate care for patients with venous insufficiency and women’s health conditions, including fibroids.
Key Responsibilities
- Conduct mobile patient evaluations across designated partner practices and affiliated surgical centers.
- Perform and interpret venous studies; assist with Varithena and other venous insufficiency procedures.
- Collaborate with OBGYN offices, vascular specialists, and referring physicians to support comprehensive patient care.
- Maintain thorough, accurate documentation in eClinicalWorks (EMR) or similar systems.
- Educate patients on procedures, treatment plans, expectations, and follow-up care.
- Work closely with internal StreamXCare scheduling teams and external partner staff to ensure smooth patient transitions.
- Provide reliable communication, consistent follow-up, and strong clinical judgment across varied care environments.
Qualifications
- Active NYS Nurse Practitioner or Physician Assistant license.
- Minimum 1 year of experience in vascular, venous, interventional radiology, or OBGYN-related settings.
- Experience with venous studies and/or venous treatments (Varithena or similar) preferred.
- Strong communication and documentation skills.
- Proficiency in eClinicalWorks (ECW) or equivalent EMR systems.
- Reliable transportation and flexibility to travel between practices within Queens and nearby boroughs.
Compensation & Benefits
- Base Salary: $150,000–$175,000 annually.
