Full Job Description
BRIEF POSITION SUMMARY: Provides health care to patients in the hospital or office setting. Works in consultation, cooperation and coordination through a collaborative physician. Functions according to the established standards and practices of medicine for Advanced Practice Practitioner in the State of New York
ESSENTIAL FUNCTIONS.
1. Assesses health conditions and collects patient data through use of means such as interviews, observation and elimination.
2. Identifies the need for additional data collection including the need for data collection from other health providers;
3. Documents findings and actions appropriately in the patient’s medical records.
4. Assists in educating the patient and family members regarding medical condition, procedures, and medication and preventative measures as necessary and appropriate.
5. Recommends community resources to meet patient and family needs.
6. Triage patient’s telephone calls and provides consultations and/or recommendations for appropriate treatment.
7. Performs evaluations and treatment procedures in emergency situations when necessary.
8. Maintains patient confidentiality.
9. Attends required meetings and. participates on committees as requested.
10. Participates in professional development activities and maintains professional affiliations and CME’s
11. Performs related work as required or assigned.

2. Must continually demonstrate the safety procedures for the clinic and utilize universal precautions where appropriate.
3. Must display an image of personal and professional competence, including knowledge and adherence to the philosophy of Continuous Quality Improvement.
QUALIFICATIONS:
1. Education: Degree and Board Certification in Nurse Practitioner or Physician Assistant is required. Experience: A minimum of 1 to 3 years work experience as an Advanced Practitioner is required.
2. Must understand general insurance protocols and applicable billing guidelines and regulations.
3. Professional verbal and written communication skills are required to establish and maintain effective working relationships with patients, physicians, team: members and the public.
4. Ability for analytical and critical thinking and solid problem solving is essential.

Pay: $105,000.00 – $125,000.00 per year
