Salary Range: $106,000-$125,000

Great Lakes Cancer Care, affiliated with General Physician, P.C. in Buffalo, has an opening for a Multidisciplinary Coordinator (Physician Assistant or Nurse Practitioner) to work as an Outpatient APP.

We are recruiting an experienced APP for our outpatient setting to work alongside our compassionate team. As a local organization, we are committed to Western New York and seek an APP with experience who would be interested in a patient population that needs your care.

The primary purpose of this position is to initiate and provide medical care in a tumor specific disease specific health clinic with a focus on care plan execution, evidence-based practice adherence, expert coaching, consultation, and collaboration, to minimize complications, to improve coordination and time to treatment and avoid deviations of the care process.

ESSENTIAL FUNCTIONS:

  • Inform patients about the program and the flow and urgency of appointment and tests that will be performed based on concern of disease.
  • Review medical records to assure all information is present and create efficient pathway for assessment and treatment; includes contacting referring physicians/PCP for medical needs that may have been discovered or referring patient to appropriate specialty/provider as needed.
  • Request outside diagnostic tests and pathology/cytology as needed.
  • Generate weekly lists of patients to be discussed at tumor board and communicate with referring physicians regarding patient referrals as needed in preparation for tumor board.
  • Order and interpret labs or diagnostic tests needed for assessment and treatment planning.
  • Prescribe medications as needed, including pre-procedure medications.
  • Oversees that patient care pathways are followed, including time targets for relevant interventions.
  • Conduct peer to peer review and write medical necessity letters for pre-authorization of tests as appropriate.
  • Perform age-appropriate history and physical examinations for complex acute, critical and chronically ill patients at new and follow up appointments.
  • Send new patient letters to referring physicians using E chart system.
  • Collaborate with interdisciplinary team members during tumor board and after to finalize patient plans and assisting with ordering tests and referrals as needed
  • Facilitate patient and family decision making by providing educational tools.
  • Accurate and efficient documentation in E chart system.
  • Deliver exemplary customer service in order to provide a positive patient experience across the organization
  • Perform other duties assigned by management.
  • Participate in research in conjunction with faculty expectations, including tracking patient outcomes/data and assisting with database accuracy and management.
  • Participate in Oncology program development to enhance the site-specific Oncology Multidisciplinary Program.
  • Participation and leadership in evidence-based practices and the development and successful execution of clinical pathways and programs through commitment to patient safety, and an enduring focus on continuous quality improvement producing excellence in patient outcomes.

The Requirements

  • Graduate of an accredited PA/NP program
  • Experience (Minimum Needed): Two years of Oncology experience, including a minimum one-year experience with solid tumor, clinical care experience required.
  • Experience (Preferred): Oncology care coordination experience. Prior experience in GI cancer, thoracic surgery/cancers, and/or breast cancer. Experience with telephone triage in oncology, thoracic, and/or GI surgery.
  • Unrestricted NY State License
  • DEA Registration
  • Prior outpatient or inpatient experience as a provider required
  • ACLS certification
  • Infection Control certificate
