Full Job Description

Key Responsibilities:

Direct Patient Care & Clinical Decision-Making (Core Duties):

  • Diagnose and manage acute and chronic non-malignant hematologic conditions under physician supervision.
  • Conduct comprehensive assessments, including physical exams and detailed patient histories.
  • Develop and adjust individualized, evidence-based treatment plans in collaboration with the supervising hematologist(s).
  • Order, interpret, and follow up on laboratory and imaging studies, including CBCs, coagulation profiles, iron studies, and hemoglobin electrophoresis.
  • Monitor treatment response and manage potential side effects from therapies such as iron infusions, anticoagulants, and factor replacement therapies.

Patient Support & Care Coordination:

  • Provide patient and family education on diagnoses, medications, treatment options, lifestyle modifications, and self-management strategies.
  • Coordinate referrals to other specialties (e.g., cardiology, genetics) as clinically appropriate.
  • Facilitate patient transitions to and from emergency care or hospital settings, including admissions and discharges.

Collaborative & Administrative Functions:

  • Collaborate with physicians, nurses, pharmacists, social workers, and other healthcare professionals to ensure comprehensive patient care.
  • Accurately document patient encounters, assessments, and treatment plans in the electronic medical record (EMR) in a timely manner.

Professional Development & Quality Improvement:

  • Participate in monthly specialty clinics, staff education sessions, case reviews, and interdisciplinary team meetings.
  • Engage in quality improvement initiatives, protocol development, and evidence-based practice updates.
  • Maintain current licensure and certifications; stay abreast of emerging trends in benign hematology through continuing education.

Qualifications:

  • Current and unrestricted Nurse Practitioner or Physician Assistant license in New York.
  • Certification as an NP by ANCC or AANP or certification as a PA by NCCPA.
  • Prescriptive authority (including DEA registration, if applicable).
  • Prior experience in hematology, oncology, or internal medicine preferred.
  • Strong clinical, diagnostic, and decision-making skills.
  • Proficient with EMR systems; excellent organizational and communication skills.

Job Type: Part-time

Pay: $72,000.00 – $97,000.00 per year

Expected hours: 22 – 30 per week
