Join the Advanced Illness Program at Maimonides Medical Center as a Nurse Practitioner or Physician Assistant Program Manager. In this pivotal role, you will oversee the day-to-day operations of the Medical Center’s Advanced Illness Management (AIM) program and Age Friendly Health System Projects. Reporting to the Director of Business Intelligence and Value-Based Management, the AVP of Advance Practice Providers, and the Social Work Director, you will drive operational excellence and ensure high-quality care delivery.
In this role, you will:
- Manage daily operations of the Advanced Illness Management (AIM) program and Age Friendly Health System initiatives.
- Collaborate with leadership to implement strategic goals and enhance patient care.
- Utilize data-driven insights to optimize program performance and outcomes.
- Foster a collaborative, patient-centered environment to support advanced illness care.
We require:
- Completion of an accredited Physician Assistant or Nurse Practitioner program.
- Current New York State licensure and registration.
- Certification: ANCC, AANP, or AAPA.
- ACLS and BCLS certification.
We prefer:
- Knowledge of palliative care principles and practices.
- Palliative care certification.
- 1-2 years of experience as a Physician Assistant or Nurse Practitioner.
- Exceptional interpersonal and English verbal/written communication skills.
- Dynamic leadership with the ability to influence and inspire teams.
- Strong problem-solving skills and understanding of cost accounting concepts.
- Proficiency in Microsoft Office (Word, Excel, Outlook).
