Job Overview – Nurse Practitioner / Physician Assistant Clinical Supervisor:Compensation: $75 – $85/hour (Dependent on experience)Location: Brooklyn, NYSchedule: Monday to Friday, 8:00 AM – 5:00 PM
TAG MedStaffing is hiring a Nurse Practitioner / Physician Assistant Clinical Supervisor in Brooklyn, NY, with our client. In this leadership role, you’ll oversee clinical operations, supervise advanced practice providers, and deliver comprehensive care for adults with complex medical needs. You will perform assessments, support urgent and transitional care, ensure accurate documentation, and coordinate with interdisciplinary teams. Ideal for NP or PA leaders focused on care quality, oversight, and community-based care, including home visits.
Responsibilities as the Nurse Practitioner / Physician Assistant Clinical Supervisor:
- Clinical Leadership: Supervise Nurse Practitioners and clinical staff at your assigned site, ensuring high-quality, coordinated care.
- Participant Care: Conduct wellness assessments, manage acute changes in condition, and provide same-day access to urgent care services.
- Transitional & Coordinated Care: Oversee post-hospital assessments, reconcile medications, and collaborate with PCPs, CHNs, and IDT teams on individualized care plans.
- Documentation & Compliance: Ensure accurate documentation, diagnosis coding, and adherence to regulatory, HIPAA, and PACE program standards.
- Home & Community Care: Conduct home visits as needed and support oversight of home care services, ensuring continuity and proper execution of care plans.
Qualifications for the Nurse Practitioner / Physician Assistant Clinical Supervisor:
- Education: Graduate of an accredited Nurse Practitioner or Physician Assistant program required.
- Certification: National board certification required (AANP, ANCC, or NCCPA).
- Licensure: Active New York State NP or PA license required, along with DEA registration and Medicare/Medicaid provider IDs.
- Experience: Minimum 3 years of clinical experience in home care, care management, geriatrics, or complex chronic disease management; PACE experience preferred.
- Technical Skills: Proficient with electronic health records and Microsoft Office, with strong documentation and clinical assessment abilities.
- Skills & Attributes: Strong communication, leadership, and multitasking skills with a patient-centered and collaborative approach.
