Program Overview
The Health and Education Alternatives for Teens (HEAT) Program was established in 1992 to offer comprehensive medical and mental health care, supportive services, and access to clinical research for HIV+ and at-risk youth, aged 13 to 29. HEAT seeks health equity and provides age and developmentally appropriate, culturally-competent HIV care for heterosexual, lesbian, gay, bisexual, and transgender, Black, and Latino youth. A community-based program of the SUNY Downstate Medical Center, HEAT provides services at the University Hospital of Brooklyn, located in one of the highest HIV/AIDS – prevalence areas in New York City, and the nation. Our full-service clinic embraces a philosophy of “one-stop shopping,” where all HIV care is provided by an interdisciplinary team, eliminating the barriers that youth often face when trying to access health care. HEAT’s adolescent-focused model puts the young person in the center of their care and involves the client in all decisions about their treatment. Currently seeking to fill the position for the LGBTQ+ Health Specialist at the HEAT Program with a Nurse Practitioner or Physician Assistant.
Responsibilities:
- Assist physician with providing ongoing medical care to HIV infected and non-infected patients.
- Participate in case management, CQI, case presentation, and general staff and supervisory meetings.
- Provide clinical supervision to case management team.
- Work with physician and nurse to prescribe and administer treatments.
- Provide information to patients relating to health status, health care and health maintenance.
- Obtain medical histories, order routine x-ray and laboratory studies.
- Order and administer diagnostic tests.
- Refer patients to specialists as needed.
- Operate and maintain medical equipment.
- Coordinate and participate in Continuous Quality Improvement meetings and other CQI activities.
- Participate in case management, case presentation, transitional, general staff and supervisory meetings.
- Perform other duties as assigned and as delineated by physician assistant or nurse practitioner role.
Minimum Requirements:
- NYS Nurse Practitioner or Physician Assistant license.
- Prior experience working with youth, people living with HIV, LGBT communities, people of color.
- 5 years of experience providing care to HIV+ clients, youth from communities of color, and women.
- Must be able to engage and retain adolescents and young adults in care.
Preferred:
- Experience providing health services to HIV+ adolescents and LGBT youth under 30 years old.
- Experience working with multi-disciplinary teams in a healthcare setting.
- Knowledge of health system technologies.
- Excellent writing skills.
- Ability to multitask.
Additional Information:
- Desired Starting Date: ASAP
- Salary Range: $100,000 – $120,000 a year – commensurate with experience
