We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) for a per diem position to provide in-home medical care to our patients.
Position Overview:
The Home Care Field NP or PA will deliver in-home care services to patients, including assessments, diagnostics, treatment planning, and health education. This role involves working closely with a multidisciplinary team to ensure a high standard of care and the effective management of chronic conditions.
Key Responsibilities:
- Conduct comprehensive in-home assessments, including medical history, physical exams, and diagnostic evaluations.
- Develop and implement individualized care plans based on patient needs, ensuring alignment with evidence-based practices.
- Manage acute and chronic conditions, including prescribing medications and therapies as appropriate.
- Provide patient and caregiver education to promote understanding of care plans, medications, and preventive health measures.
- Collaborate with interdisciplinary team members, including physicians, care coordinators, and social workers, to optimize patient outcomes.
- Document patient encounters, assessments, and treatments accurately and promptly in the electronic health record (EHR).
- Respond to patient inquiries and follow up on care plans to ensure continuity of care.
- Participate in quality improvement initiatives and adhere to all regulatory and organizational standards.
Qualifications:
- Licensure:
- Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in New York State.
- DEA license for prescribing controlled substances.
- Education:
- Master’s degree in Nursing or Physician Assistant Studies from an accredited program.
- Experience:
- Minimum of 2 years of clinical experience, preferably in-home care, primary care, or geriatrics.
- Skills:
- Strong clinical assessment and diagnostic skills.
- Excellent interpersonal and communication abilities.
- Ability to work independently and manage time effectively.
- Proficiency in using electronic health records (EHR).
- Language Skills:
- Bilingual candidates preferred (not mandatory).
Work Environment:
- Flexible per diem schedule based on patient needs.
- Travel to patient homes required (reliable transportation necessary).
