This full-time role offers the opportunity to provide comprehensive care, including chronic care management, sick visits, medication management, gynecological exams, and preventive care. Responsibilities:
- Manage chronic conditions and provide follow-up care

- Oversee medication management and monitor outcomes
- Maintain accurate patient documentation
Qualifications:
- Active Physician Assistant license in New York
- Strong team player with the ability to work autonomously
- Proficient in timely charting
