Job Summary:

OneMD, a premier multi-site primary care practice, is experiencing significant growth and seeks a motivated Physician Assistant (PA) or Family Nurse Practitioner (FNP) to join our dedicated team. This role primarily involves providing high-quality patient care at our Bayonne and Edgewater, NJ locations, focusing on continuity of care, patient follow-up, and supporting our physicians. The successful candidate will work collaboratively within a team of healthcare professionals, manage a diverse patient population, contribute to process improvement initiatives, and ensure seamless clinical coverage. This position offers an excellent opportunity to practice comprehensive family medicine in a dynamic and supportive environment.

Key Responsibilities:

  • Manage follow-up care for patients as directed by supervising physicians, ensuring continuity and adherence to treatment plans.
  • Assess patients, diagnose acute and chronic conditions, and collaborate with the lead physician to develop and implement effective patient care strategies.
  • Develop strong patient rapport through effective communication, fostering a trusting and respectful clinical environment.
  • Collaborate closely with Medical Assistants and other clinical staff to optimize daily patient flow and operational efficiency.
  • Maintain accurate, timely, and comprehensive patient records within the Electronic Medical Record (EMR) system.
  • Provide clinical coverage across designated OneMD locations during physician absences or periods of high patient volume, ensuring uninterrupted patient access to care.
  • Educate patients and families regarding diagnoses, treatment plans, medication management, necessary lifestyle adjustments, and appropriate specialist referrals.
  • Maintain a thorough understanding of and effectively utilize OneMD’s network of specialists and hospital affiliations for patient referrals.
  • Participate in quality assurance and process improvement activities aimed at enhancing patient care delivery and clinical outcomes.

Qualifications:

  • Education & Licensure:
    • Graduate of an ARC-PA accredited Physician Assistant program OR Master of Science in Nursing (MSN) or Doctorate of Nursing Practice (DNP) from an accredited Nurse Practitioner program.
    • Current, unrestricted Physician Assistant (PA-C) license OR Advanced Practice Nurse (APN) license issued by the State of New Jersey.
    • Current NCCPA certification (for PAs) OR ANCC or AANP certification (for NPs).
    • Current New Jersey Controlled Dangerous Substances (CDS) registration and federal DEA registration (or eligibility to obtain).
    • Current BLS certification required; ACLS certification may be preferred.
  • Experience:
    • Minimum of one (1) year of clinical experience as a PA or NP in a primary care, family medicine, or related ambulatory setting is preferred.
    • Highly motivated new graduates with strong clinical rotations and relevant educational background will be considered. Academic performance and the quality of clinical training are important factors for new graduate candidates.
  • Skills and Abilities:
    • Strong clinical knowledge base in primary care, including diagnosis, treatment, and pharmacology across the lifespan.
    • Demonstrated ability to manage a significant patient volume in a fast-paced environment.
    • Excellent interpersonal and communication skills, with the ability to interact effectively and compassionately with diverse patient populations and staff.
    • Proficiency in utilizing Electronic Medical Record (EMR) systems for documentation and patient management.
    • Ability to work effectively both independently and as part of a collaborative healthcare team.
    • Strong organizational skills and attention to detail.
    • Commitment to continuous learning and process improvement.
    • Bilingual proficiency in Spanish is highly desirable.
    • Familiarity with common psychiatric conditions and medications relevant to primary care management.
    • Ability to maintain professionalism and composure in high-stress situations.
