We are a well established multispecialty practice seeking an intelligent, kind-hearted APN/PA for distinctive Sub-Acute Rehabilitation facility and Hospital facility. Looking for a strong candidate to work with a Transitional Care Team in our affiliated Post Acute facility in Bergen County. Job responsibilities include treating patients with complex medical problems with goal to prevent readmissions and transition patients back to the community. Plenty of hands on teaching, training and support. Experience with hospital or post acute care highly desirable but a strong background in Med/Surgical nursing rotations and medical floor experience would suffice. Practitioner will be a part of a comprehensive care team.
Job Types: Part Time (Sub-acute rehab only on SUNDAYS) or Full Time (Sub-acute rehab 3 days per week: 1 weekday + Saturday and Sunday and additional hospitalist work)
Part-Time Pay: $65.00 – $75.00 per hour
Full-Time Salary: $130,000.00 – $150,000.00 per year

Licenses/Certifications Required:

Clinical Skills Required:
- Strong clinical knowledge and expertise in a wide range of medical conditions and treatments.
- Proficiency in using all aspects of electronic medical record systems, such as eClinicalWorks.
- Excellent communication skills to effectively interact with patients, families, and other healthcare professionals.
- Ability to provide compassionate care while maintaining professionalism and using evidenced based guidelines.
