Physician Assistant / Nurse Practitioner – Mobile Wound Care
Job Title: Full-Time or Part-Time Physician Assistant (PA) or Nurse Practitioner (NP)

We are seeking an energetic and enthusiastic Physician Assistant or Nurse Practitioner to join our team. This role is part of our Transition Care Program (TCM), focusing on supporting patients transitioning from hospital to home or other care facilities.
Join our team as a Physician Assistant or Nurse Practitioner and play a vital role in supporting our mission to provide compassionate, high-quality care to our patients through a combination of in-person and telehealth visits.
Job Responsibilities:
- Evaluating and treating patients: Perform patient assessments and treatments in accordance with clinical best practices.
- Compassionate patient care: Deliver respectful and patient-centered care aligned with Advanced Life MSO’s mission and values.
- Chronic wound management: Manage care plans for patients who are referred to chronic wound care, and ensure patients and caregivers understand the treatment plan.
- Documentation: Accurately document all patient encounters using an electronic health record (EHR) system.
- Coding and billing: Take responsibility for appropriately coding and billing for services rendered.
- Ethical and legal standards: Deliver exemplary care while adhering to all legal and ethical medical standards.
- Clinical productivity: Maintain productivity consistent with peers and industry standards for similar mobile care settings.
- Hybrid care delivery: Provide both in-person care (home visits, facilities) and through telehealth consultations.
Minimum Requirements:
Education: Minimum of a bachelor’s degree in physician assistant studies or a master’s degree in nursing for Nurse Practitioners (Required)
Licenses & Certifications:
- Active and unrestricted New Jersey medical license (Required)
- Active DEA license (Required)
- NCCPA certification (for Physician Assistants) or ANCC/AANP certification (for Nurse Practitioners) (Required)
- Active BLS Certification (Required)
Work Experience:
- Comfortable in the treatment of wound care patients (Required)
- EHR proficiency (Required)
- Prior experience in chronic wound care (Preferred but not required; training provided)
- Previous experience in chronic wound care is preferred but not mandatory. We will provide you with training in chronic wound management with protocols.
Required Skills, Knowledge, and Abilities:
- Strong clinical assessment and care planning skills.
- Excellent communication and patient education abilities.
- Ability to work independently in a mobile setting.
- Commitment to patient-centered care and ethical medical practices.
- Organizational and time management skills to manage mobile care scheduling.
Work Environment:
- Office Location: 142 State Hwy 35, Suite 106, Eatontown, NJ
- Work Hours: Monday – Friday, 8:00 AM – 5:00 PM (Flexible scheduling available for part-time roles)
- Mobile Role: Provide care at patient homes and facilities including SNFs, Assisted Living, and Memory Care.
Start Date: As soon as credentialed or April 2025, whichever is earlier.

Pay: $110,000.00 – $130,000.00 per year
