Experienced Family Nurse Practitioner or Family Medicine Physician Assistant

We are seeking a full-time Family Nurse Practitioner or Family Medicine Physician Assistant for our Nashua, NH site. Lamprey Health Care is a non-profit, federally-funded Community Health Center in southern New Hampshire.

We are looking for an experienced NP or PA who is comfortable managing a dedicated panel of primary care patients in the outpatient setting. We care for a diverse population and offer a number of resources and programs on site to aid in the care of our patients. Supports available include live interpretation, nursing support for administrative tasks including refills and callbacks, fully-integrated behavioral health and MAT programs, remote patient monitoring with chronic care management, and care coordinators to assist patients in connecting patients with community resources.

The ideal candidate will have a minimum of 2 years in a family medicine setting, FQHC experience preferred. Bilingual in Spanish or Portuguese is preferred but not required.

Responsibilities:

  • Assess the health status of the client, performs diagnosis and prescribes treatment, including via telehealth.
  • Maintain accurate and detailed patient records.
  • Promote individual professional growth and team building at Lamprey Health Care. Maintain a close working relationship with members of the Lamprey team, including providers, nurses and support staff.
  • Contribute to Lamprey Health Care’s productivity by attaining a budgeted rate of patient care visits as determined by the Provider, Chief Medical Officer, and Chief Executive Officer, as per the Lamprey Health Care budget.

Requirements:

Registered nurse with current licensure in New Hampshire, state licensure as a Nurse Practitioner and DEA registration.

Graduate of accredited Physician Assistant (PA) Program (NCCPA certification preferred), PA license in the State of New Hampshire, DEA registration. (Physician Assistant specific.)
