Full Job Description

Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester, Putnam and Ulster Counties. A pioneer Federally Qualified Health Center, Open Door provides more than 300,000 patient visits annually and serves nearly 60,000 individual patients who might not otherwise have access. Founded as a free clinic in 1972, Open Door’s mission of building healthier communities through accessible, equitable, culturally competent health care has led to site and service expansions. Open Door is seeking a full-time Family Nurse Practitioner (FNP) or Physician Assistant (PA) for its medical site in Port Chester, NY. The NP or PA will report to the site Medical Site Director.

Responsibilities

  • Provide high quality primary care to pediatric and adult groups.
  • Provide immediate patient care and, where necessary, refer the patient to a primary care team for more comprehensive care.
  • Develop and implement a plan of care in collaboration with other members of the care team, the patient, family members and significant others.
  • Ensure high quality patient care is delivered cost-effectively and consistently.
  • Provide appropriate specialist referrals, follow-up and hospitalization when indicated.
  • Maintain electronic medical records in compliance with Open Door’s standards with high efficiency.
  • Partner with Open Door’s Behavioral Health team and resources.
  • Maintain familiarity with ongoing research and development in relevant fields.
  • Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions.
  • Represent Open Door in community outreach activities.
  • If Designated as responsible for Bio Hazardous Waste removal, complete online training and receive certificate prior to completing this task.

Requirements

  • Degree and licensure to practice as a Nurse Practitioner or Physician Assistant in NY.
  • Bilingual: English and Spanish.
  • Minimum of 2 years relevant experience; experience working in emergency, urgent care or walk-in clinic desirable.
  • Outstanding “bedside manners”: patient-centric with empathy, excellent active listening and communication skills, engaging and efficient.
  • Outstanding clinical capabilities and judgment.
  • Focused on collaborative team-based care.
  • Responsive and capable of working in a high accountability environment.
  • Comfort level with using an electronic health record.
  • Flexible.
  • Excellent judgment and ability to prioritize resources where they will be most useful.
  • Comfortable receiving and providing constructive feedback.
  • Engaged in learning and continuous quality improvement.
  • Outstanding written and verbal communications skills with a culturally diverse patient and staff population.
  • Passionate about providing high quality primary health care to low-income, under-served residents and those lacking access to health care.
