Job Summary:

Vitability Health is seeking a highly skilled and experienced Physician Assistant or Nurse Practitioner to join our team. In this role, you will be responsible for providing remote medical care to seniors and patients who require remote patient monitoring. You will work collaboratively with our team of healthcare professionals to deliver exceptional care and improve health outcomes.

Responsibilities:

  • Provide remote medical consultations, diagnoses, and treatment plans for patients through telehealth platforms.
  • Conduct comprehensive patient assessments, evaluate patient histories, and order and interpret diagnostic tests.
  • Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
  • Educate patients and their families about their health conditions, treatment options, and self-management strategies.
  • Utilize remote patient monitoring technology to monitor and manage patients’ health conditions.
  • Document patient encounters accurately and in a timely manner in the electronic medical record system.
  • Stay up-to-date with best practices in telehealth and remote patient monitoring and participate in ongoing education and training programs.

Requirements:

  • Active and unrestricted license to practice as a Physician Assistant or Nurse Practitioner in the state of New York.
  • Minimum of 3 years of experience in primary care, geriatrics, or a related field.
  • Excellent communication skills and the ability to establish rapport with patients, families, and other healthcare professionals.
  • Strong clinical skills and the ability to work independently.
  • Experience with telehealth platforms and remote patient monitoring technology.
  • Familiarity with electronic medical record systems and documentation requirements.
