Job Title: Physician Assistant – Program Manager, Age-Friendly Health SystemLocation: Brooklyn, NYSchedule: Monday – Friday, 9 AM – 5 PM (40 hours/week)Salary: $130k – $150k annually, plus excellent benefits

About Us:Our Advanced Illness Program in Brooklyn, NY, is seeking an experienced Physician Assistant to lead critical initiatives in our Age-Friendly Health System. Key Responsibilities:Under the direction of the Director of Business Intelligence, the AVP of Advanced Practice Providers, and the Social Work Director, the Program Manager will manage the day-to-day operations of the Advanced Illness Management (AIM) program and lead projects supporting Age-Friendly Health System goals.

Primary Duties:

  • Oversee all operational aspects of the AIM program.
  • Develop and implement strategies to enhance care for patients with advanced illness.
  • Lead initiatives related to Age-Friendly Health System practices.
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
  • Monitor program performance and ensure the highest quality care and cost efficiency.

Requirements:

  • Completion of an accredited Physician Assistant program.
  • Current NYS licensure and registration.
  • Certification through AAPA.
  • ACLS and BCLS certifications.
  • Strong leadership, communication, and organizational skills.
  • Proficiency with MS Office (Word, Excel, Outlook).
  • Experience in problem-solving and understanding cost accounting.

Preferred Qualifications:

  • Knowledge and experience in palliative care.
  • Certification in palliative care.
  • 1-2 years of experience as a Physician Assistant.
  • Ability to lead and inspire teams to provide exceptional care.

Pay: $130,000.00 – $150,000.00 per year
