Work Schedule Options:

  • 5x8s- Saturday required. Can choose second day off!
  • 4x10s- Saturdays required. Can choose both days off!
  • Part-time Options also available.

Summary of Responsibilities

Community Health Partners is looking for a Nurse Practitioner or Physician’s Assistant with independent clinical experience in a Primary Care/ ER/ Urgent Care setting to diagnose and treat health conditions, provide patient education, collaborate with Home Health/Hospice providers and other healthcare providers for all age groups. Our NP/PA will make house calls to private residences, skilled nursing facilities, and assisted living facilities for services ranging from acute illness, routine preventative care, physical examinations, chronic disease management, post hospitalization follow- up, and care coordination referrals. The position involves conducting physical examinations, ordering and interpreting labs and other diagnostic procedures, medication prescribing and management, and overseeing patients’ whole-body health. You’ll be both a health practitioner and an educator. As our NP/PA, you will work closely with other healthcare professionals, family members, etc. thus you should have a team spirit and a positive attitude. Patience, critical thinking, and the ability to communicate both written and verbally are imperative as an in-home Primary Care provider.

Qualifications

  • Active Nurse Practitioner (NP) or Physician’s Assistant (PA) License in the state of Nebraska. Board Certified preferred.
  • Two (2) years of experience as a Nurse Practitioner or Physician’s Assistant in an inpatient or outpatient primary care setting highly preferred.
  • Ability to confidently work with youth, adult, and geriatric populations.
  • Expertise in primary care, acute care, and chronic care practices.
  • Licensed driver with an automobile that is in good working order and ability to drive various weather conditions to ensure reliability for patient care.

Pay: $85,000.00 – $150,000.00 per year
