Community Healthcare Partners – Now Hiring Part-Time APRN/NP
Community Healthcare Partners is seeking a dedicated Nurse Practitioner (NP) / Advanced Practice Registered Nurse (APRN) to provide high-quality care for patients in long-term care facilities in Lincoln and surrounding area. This will include Fremont. Mileage is paid!
We are looking for a skilled provider with primary care experience/credentialing who can confidently manage a variety of health conditions. This position combines hands-on patient care with education and collaboration alongside our experienced healthcare team.
Position Highlights

- Flexible scheduling
- $65-$75/hr Part-Time Rate
- Full-Time bonus structure: Base of $90,000+ bonus potential, earning range $150,000+ annually (uncapped)
- Facility rounding focus (not remote)
- 2-year commitment required
- Start date following credentialing
Responsibilities
- Perform physical exams and review medical histories
- Order, interpret, and follow up on diagnostic tests
- Diagnose and treat acute, chronic, and infectious conditions
- Prescribe and manage medications
- Educate patients and families on health management and prevention
- Document accurately in EMR and maintain schedules
- Collaborate with the Medical Director and interdisciplinary team
- Promote patient health and well-being through proactive care
Qualifications
- Licensed Nurse Practitioner in Nebraska (APRN/NP)
- Proven experience in Primary Care (credentialed).
- Strong diagnostic and clinical decision-making skills
- Knowledge of healthcare regulations and compliance
- Excellent communication and patient education skills
- Team-oriented, ethical, and compassionate approach
