Full Job Description
D5 Health is a longevity medicine practice serving men and women seeking sustained optimization rather than sick care. We’re looking for a PA-C or NP who practices what they preach — someone for whom longevity medicine isn’t just a career but a lifestyle.
You’ll work directly alongside Dr. Deibel delivering longevity-focused care to a discerning patient base. You’ll own your own panel, spend meaningful time with each patient, and practice medicine with rigor, relationship, and real outcomes — no 15-minute slots, no checkbox medicine.
Your scope is broad and cutting-edge: hormone optimization (TRT and HRT), peptide therapy, metabolic health and GLP-1 protocols, advanced diagnostics including body composition analysis, VO2 max testing, genetic testing, and biological age markers. You’ll also have direct access to Dr. Deibel’s national and international longevity network for advanced training and collaboration.

Qualifications
- PA-C or NP with active, unrestricted licensure
- Licensed in North Carolina or eligible to practice in NC
- 2+ years of clinical experience, ideally with exposure to hormone optimization, peptide therapy, metabolic/GLP-1 protocols, or functional medicine
- Personal commitment to longevity medicine — someone who lives what they prescribe
- Strong clinical judgment and comfort with protocol-driven but individualized care
- Entrepreneurial mindset; comfortable with autonomy
- Excellent bedside manner for an educated, engaged patient base
Compensation & Benefits
- Paid immersion period shadowing Dr. Deibel, transitioning to competitive base salary with quarterly productivity bonus
- Monthly healthcare stipend (QSEHRA)

- 15 days PTO plus federal holidays
- $1,500 annual CME allowance with paid time off to attend
- License and DEA fee reimbursement
Pay: $100,000.00 – $130,000.00 per year
